Sciatica is a common and debilitating set of symptoms which often accompany low back pain in which the patient may experience shooting leg pain and/or impaired lower limb sensations such as numbness, pins and needles, tingling and/or burning pain. The symptoms associated with sciatica can have a sudden or slow onset and can vary from mild to severe and often follow a forward bending and twisting injury to the low back.

           What causes sciatica?

Sciatica can be caused by irritation and/or compression of the nerves which travel from the lower spine down into the legs. This can be from inflammation and/or direct compression of the nerve roots usually from a disc injury and/or degenerative age-related spinal changes.

How will your osteopath diagnose sciatica?

In order for sciatica to be diagnosed appropriately your osteopath will take a full and detailed case history and will ask you questions about your back and leg pain / symptoms and will also perform a thorough physical examination including neurological evaluation.

Your osteopath in active discussion with you may sometimes need to onward refer you for further examination / investigations to rule out other possible health related conditions.

How will your osteopath help you recover from sciatica?

Sciatica usually improves and people will fully recover within 12 weeks with a combination of time, modifying activities, pain medication, manual therapy and exercise prescription. However, this can vary significantly between individuals and it is important to remember that lots of things can influence your symptom progression such as stress, poor sleep, occupational hazards and emotional and physical well-being.

Non-invasive treatments and advice your osteopath will offer you to help control and relieve your sciatica for sciatica and low back pain include:

Osteoarthritis is classified as a clinical syndrome of joint related pain, stiffness and swelling accompanied by varying degrees of functional limitation and reduced quality of life. It is the most common type of arthritis, with an estimated 7.4 million people in England over the age of 45 with it. The most commonly affected joints are the hips, knees, lumbar and cervical spine and small joints of hand and wrist. Joint related pain, stiffness, reduced function and the associated effects on an individual's ability to carry out their normal activities of daily living can be important consequences of the condition.

The latest National Institute for Health and Care Excellence (NICE) guidelines, state that physical activity should be the main treatment – not painkillers. This is in line with new draft guidelines for the NHS which have outlined that exercise should play an integral part in the successful treatment and management of individuals suffering with the symptoms associated with osteoarthritis.

If your car broke down you’d call a mechanic, if you had a leaking pipe you’d find a plumber and if your laptop stopped working, you’d seek the help of an IT specialist.

When it’s comes to your musculoskeletal system, don’t wait for a full blown break down, call the Osteopath when the warning lights come up on the dashboard. Get an appointment booked sooner rather than later. This will allow the injury to heal quicker and it will also mean fewer visits! Less pain for the patient and purse!

Back Pain: Prevention is better than cure

Back pain causes 5 million people to consult their doctor every year and will cause pain to 1 in 5 of us at some point in our lives. Find some simple day to day exercises to help prevent it.

Back pain causes 5 million people to consult their doctor every year and will cause pain to 1 in 5 of us at some point in our lives.

Back pain causes 5 million people to consult their doctor every year and will cause pain to 1 in 5 of us at some point in our lives.

In most cases back pain comes on slowly, starting as an ache, reduced movement and can also include shooting pains into the legs. It can be caused by many different factors including arthritis, trauma, slipped discs or sciatica - leg pain due to trapped nerves. More recently it has been linked to people having increasingly sedentary lives (especially as a lot of people now have their workstations set-up poorly at home).

More and more doctors are left with limited options (such as medications and physiotherapy for which there is often a long waiting list with which to help patients) so the patient is left trying different therapists and therapies for a cure.

Research seems to show that surgery is not a quick fix for back pain and is only necessary in less than 4% of cases. It indicates that prevention is indeed better than cure and that exercise and good posture are very important, particularly in conjunction with a therapy such as Osteopathy.

Weak stomach and back muscles mixed with a sedentary lifestyle is a recipe for bad backs. Exercise strengthens the core muscles of the abdomen and spine and eases the tension in the back. Any exercise is good, although it is best to avoid high-impact sport if you have just had an episode of back pain – perhaps stick to walking, cycling and cross-training for example. Try and aim for at least 30 minutes of exercise 3 times per week where you are getting your heart pumping a bit harder than when you are at rest (you are working too hard if you are not able to talk while you are exercising). One session per week of pilates or yoga (classes are available online – contact me for more information) is also recommended as an ongoing means to avoiding back pain. Both are very effective, although it always important to proceed through each exercise slowly if you are a beginner and only attend classes with teachers who are moving around the room and watching that you are doing the exercises correctly. One to one classes are the ideal way to get personalised instruction and introduce you to exercises at your pace.

Start the day with some gentle knee hugs and stretches and then make a habit of taking regular breaks from your computer and, if you are very sedentary – every 45 minutes or so, perhaps doing some of the exercises below to keep yourself pain-free.

             Neck stretch

            - reach down and hold on to the side of the chair with the right hand while gently tilting your head to the left. Feel the stretch down the right side of your neck and shoulder. Hold for 20 seconds and repeat on the other side.

             Chest stretch

            - using a resistance band (which can be bought at most department stores), hold the band over in a wide grip over the head. Take the arms back so that you stretch out the front of the chest. Hold for 20 seconds. Avoid if you have shoulder problems.

             Spinal twist

            - with your feet flat on the floor, tighten the abdominal muscles and gently twist your upper body. Only twist as far as is comfortable and keep your back straight and hips facing forward. You can hold onto the side of the chair to deepen the stretch. Hold for 25 seconds and repeat on the other side.

             Shoulder rolls

            - take a breath in and then roll your shoulders backwards and down while exhaling. Take another breath in and then exhale while rolling your shoulders forward. Repeat 5 times.

             Upper back stretches

            - Stretch the arms out and rotate them so that the palms face away from each other. Cross the arms so that the palms are pressed together or lace your fingers together if this is uncomfortable. Tighten the abdominal muscles and round the back, reaching away as you relax the head and shoulders. Imagine you are curving over a ball. Hold for 30 seconds.

Lace your fingers together and stretch them to the ceiling. Take a deep breath in as you stretch up as high as you can. Stretch to either side and then exhale, open the arms and sweep them back down. Repeat 8 times.

What is an Osteopath and what do they do?

Osteopathy is a system of medicine that focuses on treating, and preventing, painful muscle and joint problems. By using a range of hands-on-treatments, Osteopaths focus on creating balance within the body.

Osteopaths use joint mobilisations and manipulations (the 'click') together with stretching and massage techniques to increase the flexibility of joints and body tissues. This helps to relieve muscle tension, relieve muscle spasms and improve tissue health, all of which aid the body to heal itself.

Osteopaths are commonly thought of for just treating back pain, and while this is a big part of an Osteopath’s work, Osteopathy can also be used to treat a variety of other ailments such as:

What are the differences between Osteopaths and Chiropractors?

The founding of osteopathy (by Dr Still) pre-dates chiropractic treatment by twenty one years.

Chiropractors tend to focus mainly on the alignment of the spinal column, whereas osteopaths assess the whole musculoskeletal system and help improve its function by correcting the overall posture and alignment.

Osteopaths can treat a wider range of ailments, including issues associated with the respiratory and digestive system.

Chiropractors rely on diagnostic imaging such as X-rays, whereas osteopaths will firstly perform a clinical examination, and will generally refer patients for more diagnostic procedures if required.

Osteopaths tend to use a greater variety of techniques to influence the body’s own healing system such as muscle and soft tissue techniques, joint articulation and manipulation, whereas chiropractors’ primary tool is the vertebral “adjustment”, which is similar to osteopathic manipulation.

Chiropractic appointments tend to be shorter as they primarily focus on adjustment techniques which are quicker to carry out. Osteopaths spend more time with their patients per visit as their approach is broader and treatments tend to be spaced out over a longer period of time.

Chiropractors tend to need to see their patients more frequently since the adjustments are usually only temporary and the muscles tend to pull the misaligned vertebrae out of position again. This means that more adjustments are needed for them to settle into position.
